Director, Connected Communities of Care will be responsible for successful implementation of PCCI’s community-based programs that are funded by federal and local governments as well as by other partner philanthropic organizations. This will include successful deployment of programmatic and technical aspects of these programs and ownership of PCCI’s relationship with CMS and other governmental and non-governmental funding agencies.

Education: Must have Bachelor’s degree in Public Health, Health Economics , MHA or related degree. Advanced degree preferred.

Experience: Five (5) years of supervisory experience required. Must have successfully deployed at least one multi-year, complex project, in the healthcare domain with a mix of clinical/community, operational and technical aspects. The project should have involved managing direct and in-direct reports, executing on specific milestones and targets, managing budgets and developing partnerships with a variety of external stakeholders and partners. Experience delivering multi-year government (CMS, CMMI, NIH, etc.) grants with direct responsibility.
